We approach care coordination services holistically by helping clients access medical assistance, mental health resources, substance use treatment, and health insurance enrollment.

Health Home Care Coordination

Since 2012, NADAP has participated in New York State’s Health Home Care Management initiative. We now operate the program throughout New York City, and in Nassau and Suffolk counties. Medicaid enrollees living with the following health conditions may qualify for free, voluntary healthcare coordination:

Two or more chronic conditions, such as:
• Diabetes
• Asthma
• High Blood Pressure
• Heart Disease
• Substance Use Disorder
• Depression

Or

• HIV/AIDS

Or

A Serious Mental Illness, such as:
• Major Depressive Disorder
• Bipolar Disorder
• Schizophrenia

 

Care Coordination focuses on the healthcare needs and preferences of each person served. NADAP’s Care Managers provide the extra support to help clients find and access medical, mental health, substance use disorder, and social services to improve their health and other life areas that impact their wellbeing. In a highly individualized manner, they
• identify and make referrals to service providers,
• assist with scheduling and getting to appointments,
• help clients understand and follow recommended medications and treatment, and
• ensure that care from different providers is well coordinated and lead to better health outcomes.

Our Care Coordination team further offers:
• Custom outreach and engagement strategies dependent on the individual’s needs, goals, past treatments, and preferences.
• Comprehensive assessment of the individual’s current and previous medical, mental wellness, social, legal, and housing status.
• Assistance with Health Home member benefits, such as Medicaid applications and recertification as well as for public assistance, SSI, SSDI, housing, employment, and other benefits and services.

Care Managers work in the community and they meet clients at their home, at their doctor’s office, or other venues of the person’s preference. Ultimately, the ongoing coordination of a person’s care helps prevent emergencies, reduces hospital visits, and improves the participant’s overall health status.

NADAP was honored to receive a visit from InUnity Alliance’s (IUA) Executive Team, Jihoon Kim, Amanda Semidey and Sarah DuVall at our flagship Grand Central location.

After a tour of our offices and a peek into 2 Vocational and Employment workshops, we discussed all the various programs that NADAP offers that help 35,000 underserved individuals annually, over lunch.

Earlier this year, we collaborated with InUnity Alliance to help restore $11.4M in NY State funding for Vocational and Job Placement Services, providing vital support to individuals in recovery.

Our guests from IUA got to see first-hand, the importance of these essential Vocational and Job Placement Services to individuals with SUD in the recovery process.

🎉 We recently celebrated National Recovery Month, recognizing the 21+ million Americans in recovery. Our staff from all 6 NADAP offices came together at NADAP’s headquarters for a night of recognition, connection, and fun!

🙌 CEO John Darin highlighted our unwavering commitment to “serving clients with dignity and professionalism.” We also honored employees who provide outstanding service to our clients, shining a spotlight on their dedication.

🎁 The evening was filled with delicious food, laughter, and exciting activities. Our “Who Is the Boss?” game had everyone guessing, and all 4 supervisors were identified! Congrats to Cheri, the lucky winner of the big raffle prize!

💜 We cherish the tradition of hosting this party every September as part of National Recovery Month.
